Lettie will need to be informed by the professionals about what is going to happen in her case. By doing this will help professionals at practice to know the concerns Lettie has. Lettie will be giving the choice to consent to any procedures going to be performed.  
 
CONTINUITY OF CARE:  
For Lettie, continuity of care will start from going to the recovery ward after her C-section, through to the maternity ward and then her discharge to go home. In community, Lettie will have the midwife’s visits, health visitor, social workers, Gp’s and other professionals in community that can continue with her care. This will give her the best continuity of care with the confident that she is protected from any further domestic violent, the welfare of her children is at peace and her health is being looked after well.  
 
IMPROVE EXPERIENCE OF CARE:  
Stewart et al. (2000) suggested that, Patient-centered practice improves health status and increases efficiency of care by reducing diagnostic tests and referrals”. 
Lettie will have a positive mind set, self-confident, self-determination about the service she had received in health care and what she will be receiving in the future.  
 
PHYSICAL, CULTURAL, AND PSYCHOSOCIAL ENVIRONMENT OF HEALTH SERVICES SUPPORTS PERSON-CENTERED CARE.  
Mcmillan, S. Et.al (2013) stated, Holistic care: Recognizes and values the whole person and responds to his or her complete needs in context, that is, simultaneous assessment of physical, cognitive, and psychosocial functioning, addressing situational, family, cultural, and/or religious needs. 
An example of this is in our scenario. Lettie is a Jehovah’s Witness. Who is going to a theatre for a C-section. In Jehovah witness religion, they have a different view of blood transfusion and therefore the professionals working together will need to treat her religion with respect and to make sure they have her consent just in case a situation arises.  
 
BENEFITS OF A SERVICE PROVIDER 
It broadens knowledge and skills. Working together will help professionals to work well and learn from one another. This will also help each professional to improve upon their knowledge and skills and to also give the best patient centered care.

when team work is effective, and success has been achieved after practice. It encourages professionals to boldly come forward to lead a team when in practice.

Better outcome will help each professional to feel proud of themselves and what they have achieved. This will increase team effectiveness and it will have a positive impact on patient care.
